Our home health care services include but are not limited to personal care, homemaking, and companion services. Our services are provided in the client's place of residence and are designed to support the client's independence and well-being.

Eligibility:

To be eligible for our home health care services, clients must be over the age of 18, live in their place of residence, and require assistance with daily living activities.

Service Agreement:

Our services are provided based on a service agreement between the client and [Company Name]. Limitation of Liability:

Dispute Resolution:

Any disputes arising from the provision of home health care services will be resolved through mediation. If mediation is unsuccessful, the parties may seek resolution through arbitration.

Governing Law:

These terms and conditions will be governed by and construed in accordance with the laws of the State of Minnesota.

Entire Agreement:

These terms and conditions constitute the entire agreement between Creative Living Group and the client and supersede all prior negotiations, understandings, and agreements between the parties.
